Surrender

Due to an increase in surrender inquires, we now require a surrender form to be completed prior to our shelter accepting your animal.

We are limited in kennel space and reserve the right to decline your animal if we are unable to accept them into our care. Please complete the surrender form with as much detail as possible

Please allow 7-10 days for your form to be reviewed and message us on Facebook or call 570-333-5265 to check the status.

IF YOUR ANIMAL WAS ORIGINALLY ADOPTED FROM BLUE CHIP, PLEASE COMPLETE THE RETURN FORM.

Animals adopted from our shelter must be returned to us if you can no longer care for them
